How the Practice uses Personal Health Information

Confidentiality is of the utmost importance to the doctors. Access to information we hold about patients is limited to staff who are involved in your care and treatment. Information is stored securely on the computer and in medical records. Sometimes it is necessary to share this information with other organisations involved in your healthcare eg when you are referred to hospital. No information is passed to non-clinical organisations outside the surgery without patient consent. Please read our Privacy Notice.

Patient Rights and Responsibilities

These are summarised on this website. We aim to give you the best possible care and attention at all times, treating you with respect and courtesy. Practice staff expect the same courtesy and cooperation from patients. Please let us know if you cannot keep an appointment and if you change your address and/or telephone number.

Zero Tolerence

We follow the Hounslow Primary Care Trust guidelines on zero tolerance with regard to verbal, threatening or physical abuse. This may include removal of a patient from our register or prosecution of offenders.

Data Protection

Under the Data Protection Act 1998, patients may have access to, and copies of, their medical records (subject to certain circumstances when information may be withheld). Requests should normally be made in writing and the Practice will provide the information within 21 days. A fee may be chargeable. Please ask at reception for further details. Patients are entitled to request that any perceived inaccuracies be corrected.
